I am currently only accepting the following insurance plans for patients in Washington State: Premera
If you do not have one of the insurance plans I am in-network with, you can still see me as an 'out-of-network' provider. I can provide you with a Superbill to submit to your insurance company if they offer out-of-network benefits for reimbursement. Please note that I am unable to provide Superbills for Medicaid or Medicare. If you’d like, I can also provide you with a breakdown of the CPT codes and fees I use, so you can contact your insurance company directly to inquire about potential reimbursement.
Let me know if you'd like further adjustments.
You can use your insurance to cover medications I prescribe to your pharmacy and any labs I order, just as you would for any other healthcare services.
The typical fee structure is as follows:
*Please note that fees are subject to change.
For any professional services that take longer than 10 minutes, such as report writing, phone calls, treatment summary preparation, communication with family or other providers, or any other services performed on your behalf, there will be a charge of $50 for each 10-minute increment. Insurance typically does not cover these services. Any services that take less than 10 minutes will not incur a charge, and there is no charge for brief email communications. If a service is expected to take longer than 10 minutes, I will inform you in advance before the charge is applied.
